• Full Name of the Individual
• Office/Institution Name and Address (if applicable)

3. Verification

• All documents must be original at the time of verification.
• Photocopies may be collected for clinic records.
• Glory Eye Clinic Optical reserves the right to confirm the validity of documents with the issuing authority.
